Starport is excited to announce our reinvigorated Personal Training Program for 2012. Now you can train in groups of two or three. Training with a friend can help keep you both on track to maintain your fitness routines and can save a little money at the same time.

| How to Register |
|
- Pick up a Personal Training Package Questionnaire at the Gilruth Information Desk or download a Training Package Questionnaire

- Complete the package and drop it off at the Gilruth Information Desk
|
- After reviewing your file, a Gilruth Representive will contact you to schedule your comprehensive fitness assessment. The Comprehensive Fitness Assessment includes blood pressure screening, body composition assessment, flexibility test, cardiovascular endurance analysis, and muscular endurance test. Our Personal Trainers will also conduct an analysis of health history, exercise history, and fitness goals.

- Payment for Personal Training can be made with cash, check, VISA, Discovery or MasterCard and is due before your first sessions or as stated on your payment plan.

| Session Duration |
- All personal training sessions last one hour.
|
- NOTE: Your first training session will consist of a Comprehensive Fitness Assessment. All sessions thereafter will be hands-on training sessions.

- Late Policy: All clients are urged to arrive for their session on time. Sessions that begin late will still end one hour after the scheduled start time. Clients arriving 30 or more minutes late for a session will be charged for the entire session.

- Cancellation Policy for Individual Sessions: When a client must cancel a training session, please notify the Gilruth Information Desk at 281-483-0304 or your Personal Trainer at least 24 hours in advance. Failure to notify as required will result in the client being charged for the full session.

- Cancellation Policy for Group Training Sessions:
- More than 24 hours notice: In the instance that one client is unable to attend a scheduled group training session, the other individual(s) have the option to continue to train at the reserved time at Starport's posted Individual or 2-Person Group Training Session rate as applicable, or the group may reschedule their session with the trainer.
- Less than 24 hours notice: In the instance that one client is unable to attend a scheduled group training session, he/she will be charged for the full session and the other individual(s) must still attend the scheduled training session. In the instance that all individuals are unable to attend a scheduled group training session all individuals will be charged for the full session.
- If cancellations become a recurring issue, Starport Management reserves the right to terminate any further group training arrangements as necessary.
